Please Bring ALL Your medications s to your next appointment.

This includes:
-Prescription medication-Medications you buy without a prescription (like Tylenol® or cold medicine).
-Ointments or creams.-Bottles of drops.-Inhalers-Vitamins and herbal medicines.

Appointment Arrival

​Upon arriving for your appointment at Dr. Lacroix's home office, please proceed directly to the front door. We kindly request that you ring the doorbell to announce your arrival. We ask that you either bring slippers or clean shoes. Should no one answer the door immediately, this indicates that Dr. Lacroix is currently engaged with another patient and appreciates your understanding. In this instance, we ask that you return to your vehicle and wait comfortably. Dr. Lacroix will personally come out to greet you and escort you in once he has concluded his previous appointment. We appreciate your patience and cooperation in helping us maintain a smooth and respectful environment for all our patients. Please remove shoes and wear socks or slippers

Dr. Lacroix’s home office does not currently have a designated handicap ramp for entry. We understand that this may present a challenge for individuals with mobility limitations, and we sincerely apologize for any inconvenience. We are committed to ensuring accessibility for all our visitors. Therefore, we kindly request that if you have mobility issues, please inform us in advance of your arrival, or upon your arrival, so we can arrange appropriate assistance and alternative entry methods. We will do our utmost to accommodate your needs and make your visit as comfortable as possible.

We endeavour to be as on-time as possible, as we know your time is valuable. Due to the unpredictable nature of urgent visits, we may occasionally be running behind depending on the volume of fit-ins for the day, and appreciate your patience in these instances. If you have multiple health concerns, please identify all of them to our office prior to your visit, so that we can prioritize what needs to be addressed most urgently.

Planning is Key: Identify and Disclose the Form Early
If your appointment with Dr. Lacroix involves completing a form, please let us know when you book. Some forms are quite detailed and require significant time and attention. Knowing in advance allows us to allocate the necessary time and potentially advise you to schedule a dedicated appointment specifically for form completion.

Your Responsibility: Pre-Completing Your Section
Before requesting Dr. Lacroix to complete your form, please ensure you've filled out all sections that require your information. This significantly speeds up the process and allows Dr. Lacroix to focus solely on the medical information required.

Potential Fees for Form Completion
Please be aware that completing certain forms may be subject to a fee. This reflects the time, expertise, and administrative overhead involved in reviewing and completing these documents accurately.

Allow Ample Time for Completion
Please allow adequate time before your form is due to have it completed. Not all forms can be easily completed on the same day as your appointment. Depending on the complexity of the form, Dr. Lacroix may need to complete it outside of office hours. In such cases, you may be asked to return to our office to pick it up.

Due to the large volume of paperwork we process, form completion may take up to 30 days. We understand the time sensitivity of these documents and will always endeavor to complete them as quickly as possible.
