NEW PATIENTS
Thank you for your interest in becoming a patient of Dr. Lacroix. Dr. Lacroix is currently accepting new patients residing in the Alexandria, Moose Creek, Cornwall area and neighbouring communities.
We understand the importance of finding a healthcare provider you trust, and we appreciate you considering our practice. However, due to the high volume of applications we have recently received, we are temporarily putting new applications on hold. This will allow us the necessary time to thoroughly process the existing submissions and ensure each applicant receives the attention they deserve.

If you wish to be placed on Dr. Lacroix's wait list, please follow the outlined steps below. Before proceeding, please note that Dr. Lacroix is currently only accepting patients residing in the Cornwall area and surrounding nearby towns. Furthermore, Dr. Lacroix no longer provides pediatric care and therefore cannot accept patients under the age of 7. Additionally, Dr. Lacroix does not practice gynaecology. For any female gynecological concerns, a referral to a qualified gynecologist will be arranged.

To register as a new patient, you must:
1) Fill out the new patient registration form.
2) Please print the medical release form and complete it with your personal information. We kindly request that you contact your previous family physician and request that your medical files be sent directly to our office.If your previous physician requires you to pick up the files yourself, please notify our office as soon as you've done so.

Once you have confirmed that your medical records have been sent, please let us know so we can ensure they are received and properly integrated into your file.
**Please be advised that we are unable to accept medical records submitted in paper format. For efficient and secure handling of your information, we can only accept medical charts provided on a USB drive. If you currently possess paper copies of your medical records, we recommend visiting a local Staples store. Their staff can assist you in scanning your documents and saving them onto a USB drive for you. This will allow us to seamlessly integrate your records into our electronic system and provide you with the best possible care.
3 You must provide a picture copy of your "Health card" See link below. If you are unable to attach your health card, please email a copy to drlacroix@moosecreekmed.ca.
Once you have complete all 3 steps, we ask that you book your Meet & Greet Appointment in approximately four to six weeks time (not before then) to allow sufficient time for medical record retrieval.
If you require any medication renewals, please call your pharmacy and have them fax a prescription renewal request to our office.

**We kindly request that you refrain from calling or emailing our office to check on your new patient status. Our team is diligently working through each application, and processing times can take up to 60 business days. Please be assured that we will contact you directly once a decision has been made. We appreciate your cooperation in not contacting us regarding application status, as responding to these inquiries diverts valuable time and resources away from attending to critical medical matters and patient care. Your understanding and patience are greatly appreciated.**
