Please remember to bring your bloodwork requisition and health card to all appointments. If you don't have access to a printer, you can easily upload your requisition on Dynacare's website and email it to Lifelabs.

Create one email per patient

Email a PDF copy of the requisition to the dedicated email address

The requisition will be processed within 24 hours of submission
